(IFSP) Responsibilities: · Participates as a member of the transdisciplinary team in program planning and delivery of services.· Participates in intake/screening of new clients.· Participates in initial assessments and annual evaluations of children and families contributing to the development of IFSP.· Maintains safety and medical standards according to agency policy and procedures.· Maintains expected level of productivity.· Provides case management and referral services to assigned caseload, including participation in interagency case conferences.· Provides home visits to assigned caseload, for purposes of providing developmental stimulation, parent training and support.· Responsible for implementation of IFSPs· Participates in parent/child and/or child groups.· Provides co-treatments with other team members home and center-based.· Provides parent education activities.· Provides teaching and consultation to team members.· Completes narrative reports of assessments, progress reports, IFSPs, and discharge summaries.· Maintains appropriate communication with physicians, clinics, and community agencies participating in each child’s care.· Coordinates transition/termination process for clients, including referrals to Chapter 766 services when appropriate.· Maintains assigned client records according to program procedures.· Demonstrates the ability to work collaboratively in a team approach to service provision.· Maintains compliance with all Kennedy-Donovan Center policies and procedures, OCCS/DPH regulations and requirements.· Participates in professional development activities and training necessary to maintain relevant certification/licensure in accordance with their professional discipline and in accordance with EI Operational Standards.· Participates in inservice training, team meetings, group and individual supervision.· Assists in outreach and program fund-raising activities.· Performs other job-related responsibilities as assigned. | ||||
